Dear Patients.

To begin, I would like to thank you for the trust you have given me over the years as your physician.  Taking care of you and your family has been an honor for my staff and me.  Your continuing wellness and health are priorities for us.  I am writing today to inform you that I will be closing my practice, Lower Highlands Dermatology effective as of 12-9-2024.  I will be available until that date for your urgent dermatology needs. 

 In order to help you find another doctor, the following information is provided:

Another option is contacting your healthcare insurer for a referral within your network.

Of course, you may choose any provider, and you are not limited to the facilities or individuals listed here. It will be up to you to take steps to connect with a provider of your choosing and we recommend you do this as soon as possible.

Records may be requested using a records release form found on our website (lohiderm.com) between now and the end of the year.  After closing, your health records from this Practice will be transferred to Morgan Records Management, LLC (the “Records Custodian”).  The Records Custodian will continue to maintain your records in accordance with applicable confidentiality and security standards and with other applicable laws.  If you wish to have a copy of your records sent to you or to another provider or facility, you may submit a written request to this office at this time, or to the Records Custodian after the Effective Date. Requests to the Records Custodian, after the Effective Date should be directed to:

Please Note:

• In accordance with applicable state law, you may be charged for the copying and transmittal of the records.
• There may be a transition period from the time I close to the time Morgan Records Management has access to my charts.
